COMMUNITY FIRST BANK SCHOLARSHIP APPLICATION

GENERAL INFORMATION

Community First Bank (CFB) is pleased to provide $500 scholarships to high school seniors graduating from these area school districts in our general market area: Reedsburg, Boscobel, Richland Center, Livingston, Fennimore, Muscoda/Riverdale, Baraboo, Platteville, Prairie du Chien, Lancaster, Dodgeville, Mineral Point, North Crawford, Wisconsin Dells, River Ridge, Wauzeka-Steuben, Highland, Ithaca, Weston and Belmont. In addition, $500 scholarships are available to incoming or current students of any campus of these colleges studying in the areas of agriculture or business: Southwest Technical College, Madison College, and University of Wisconsin – Platteville.

The scholarship criteria is based on academics, activities, leadership, achievements, accomplishments, and written communication.

ELIGIBILITY REQUIREMENTS FOR APPLICATION

A graduating high school senior residing in our general market area with a grade point average (GPA) of 2.00 or greater through the 7th semester of senior year. We encourage all eligible students to apply; however, student(s) receiving the Academic Excellence Higher Education Scholarship will not qualify for the Community First Bank scholarship.

INSTRUCTIONS FOR APPLICATION

Please complete your personal information and the Scholarship Application Form below. The application must be completed in one session, so please review the application, gather needed information, prepare your written statement, and then return to this page to submit your information when ready.

Attach your GPA attestation/transcript and typed written communication at the end of this application. If you are unable to complete the application electronically, please reproduce the application in a similar format, including the GPA attestation and typed written communication, and return the entire packet to:

All scholarships must be received, whether submitted electronically or hard copy, by 5pm on Friday, March 14th, 2025. Late or incomplete applications will not be considered.

DISBURSEMENT

Selected scholarship recipients will be notified in early April via the email provided on the application. A check will be mailed by August 1st, 2025, to the financial aid office at the college you will be attending.

  • List the extra-curricular, community and other volunteer activities in which you were involved during high school and the number of years involved. Example: Football 4 yrs, Student Council 2 yrs, Chorus 3 yrs, 4-H 3 yrs, FCA 2 yrs, LEO Club 1 yr, Church Lector 2 yrs.

  • What leadership roles did you take and what do you think they taught you about being a leader? Example: President, Chairperson, Captain, Shift Leader, Employee of the Month. Please note that this could also apply to your outside work, work study, church or other organizations.

  • ACHIEVEMENTS / ACCOMPLISHMENTS

  • List what you feel your major achievements or accomplishments were while in high school. Example: Honor roll, letter winner, all-conference, district/state competition in forensics, mock trial, music, etc.

  • Briefly explain what you gained from your high school experiences and what you might have done differently.

  • WRITTEN COMMUNICATIONS

  • In a half-page typed document, briefly explain why you feel you need or have earned this scholarship. Example: Your strengths, goals, and special circumstances, such as, other siblings in college, single parent home, illness or disability of self or family member, etc.
